​Patients that have previously been seen at our practice can select the existing patient exam and have their information forwarded. New patients please arrive 15 minutes before your exam to fill out your new patient paperwork.

If your exam includes a contact lens prescription please be aware that there is an additional fee of 100-175 dollars for new patients and 60 dollars for existing patients that is typically not covered by your health insurance. ​
